Forms

Please register for tryouts this fall through the Badger Regions online registration program. Signup To Play (STP) www.signuptoplay.com.
Please follow all instructions on STP.

The forms must still be printed and mailed to Southwest Volleyball Club. The USAV membership and medical waiver forms must be completely filled out and signed by both parent and player prior to trying out.

The USAV membership form must be signed on the front and back.

The forms may be turned in at tryouts; however the fee is $35 if paid at the door. Atheletes should attend both tryout dates. Please arrive 30 minutes early for registration.
